What is Intensive Case Management?

"The Intensive Case Managers work with women and children who have been impacted by family violence providing case coordination. Using a strengths-based approach they work with women and children and build needs and risk assessments and create a case plan that respond to these."

What happens?

"The intake worker will talk to you and help you make a plan for your needs. If they identify that you need Intensive Case Management, then you will be allocated a Case Manager to work with you to build a plan. This plan is based on your needs, and you will be part of building it. The plan will look at your safety and that of your family and help you to put things in place to keep you and your family safe, it will also identify your other needs and assist you to find help for these. Help may include a range of things but sometimes includes housing support, assistance getting food, attending appointments, connecting you to legal advice, and more."

  • 1. Individual Case Management:
  • Person-centered Case plans in full consultation with you that encompasses emotional, social, and practical support for you.
  • Offering one-on-one sessions to address personal challenges, goals, and growth areas.
  • Risk assessments and safety planning.

  • 2. Therapeutic Services:
  • Providing access to culturally sensitive therapeutic interventions.
  • Facilitating healing circles and supportive group sessions rooted in Aboriginal traditions.

  • 3. Legal and Advocacy Support:
  • Assisting in navigating through the legal system, particularly in family violence matters.
  • Advocating for women and children within various systems to ensure fair and just treatment, including advocacy with child protection.

  • 4. Crisis Intervention:
  • Offering immediate, compassionate support during times of crisis.
  • Providing emergency assistance and coordinating with other services as needed.

  • 5. Community Engagement:
  • Involving women and their families in community events, cultural activities, and social gatherings to foster a sense of belonging and solidarity.
  • Promoting active participation in community development and cultural preservation initiatives.

  • 6. Family Reintegration Support:
  • Assisting women in rebuilding connections
  • Offering ongoing support to ensure sustainable, positive changes in familial relationships.

  • 7. Referral and Collaboration:
  • Collaborating with other service providers to ensure comprehensive, multifaceted support.
  • Facilitating referrals to specialized services as per individual needs, such as substance abuse programs or employment services.
